Firstly, the vacuum needs to have a carpet attachment. It is like a brush that rolls around the inside and believe me, if it's working properly it will get that hair up.

Second, you shouldn't sweep the hard wood floors, you should vacuum it on the floor setting.

Third, you may be brushing your dog all the time, but if you're not actually PULLING a tremendous amount of fur from her then it will all just eventually end up in your house. A new brush could solve this problem. Get the Furminator (http://www.furminator.com/). It is pricey but it is the best brush ever invented. You will not regret it, it will pull all that fur off of your dog which you can then put in the garbage or a bag. You'll be amazed.
